What we need from you

Before you can start working with us, we need three things on file:

Certificate of Insurance
Current COI naming Pursuit Restoration, LLC as certificate holder. This protects both of us if something goes wrong on a job site — it's non-negotiable before we can send you work.
W-9
A signed, current-year W-9 lets our accounting team pay you correctly and issue your 1099 at year-end without delays. No W-9 on file means no payment on file.
Company Contact Info
Basic details about your business and a go-to contact so our project managers know exactly who to call when a job comes up.
Have your COI and W-9 ready before you start — you'll upload both as part of the form below.
